④ Fees and Collection Method
For each request, a fee of 1,100 yen (including tax) must be transferred to the designated bank account.
Please note that all other expenses incurred for the request, including printing/copying costs, fees for obtaining identity verification documents, postage, and other related costs, shall be borne by the individual. Additionally, please be aware that if the fee is insufficient, we cannot process the disclosure or notification of purpose of use.⑤ Method of Response to Disclosure Requests
We will respond in writing to the address provided by the requester. For any questions, complaints, or consultations, please contact the above (8)① [Complaints and Consultation Desk].⑥ Purpose of Use for Personal Information Obtained Through Disclosure Requests
Personal information obtained in response to disclosure requests will be handled only to the extent necessary for processing such requests. Submitted documents will be retained for one year from the date the response to the disclosure request is completed, after which they will be destroyed.⑦ Reasons for Non-Disclosure
Disclosure will be denied in the following cases. If non-disclosure is decided, notification will be provided stating the reason.- When the address on the application form, the address on the identity verification document, or the registered address with our hospital cannot be confirmed as matching, making it impossible to verify the request is from the individual themselves.
- When applying through an agent, if the agent's authority cannot be confirmed.
- When there are obvious falsehoods in the prescribed application documents.
- When required documents or other materials are missing.
- When the information provided in the request form is insufficient to identify the individual.
- When the request is made outside the procedures established by our institution.
- When the subject of the disclosure request does not fall under the category of retained personal data or third-party provision records as defined in Article 16 of the Act.
- When there is a risk of harming the life, body, property, or other rights and interests of the individual or a third party.
- When there is a risk of significantly hindering the proper execution of our institution's operations.
- When it would violate other laws or regulations.
